Capacity Building Training Request
NMAETC is committed to working with its partners to facilitate strengthened and sustainable enhancement in the capacity of health care providers and organizations to provide services to HIV-infected members of racial/ethnic minorities.
NMAETC provides Capacity Building Assistance (CBA) that aims to achieve:
- Improved quality and coordination of services
- Improved efficiency and effectiveness of the organizational infrastructure
- Increased provider capacity to offer care that is culturally competent
- Increase in the type of services offered by the provider to patients that are HIV positive
Completion of this form accomplishes the following:
- Provides our staff with information that will assess the needs of your organization.
- Enables NMAETC to develop a customized list of services that would be most beneficial to your organization
- Provides information to NMAETC that highlights potential needs for CBA among providers
- Serves as official documentation of your interest in CBA services provided by NMAETC
Instructions:
- It is critical that you complete each item as accurately as possible. Please obtain the requested information from your organization’s most reliable source(s). We will keep your information in the strictest of confidence.
- Please answer the questions for your organization--including all of its sites (for organizations with multiple clinic sites) that provide HIV/AIDS-related services. If your organization is part of a larger entity that provides services other than HIV/AIDS treatment only provide the information that pertains to the HIV/AIDS-related services within your organization.
- You will be contacted by an NMAETC representative to participate in an in-person/phone interview regarding your responses to this form.
